Job Description
We are hiring a Family Health Educator to support families with health promotion, preventive care education, and care coordination. You will develop culturally sensitive health workshops, conduct home visits, assess needs, create care plans, and connect families with community resources. Required: bachelor’s degree in public health, nursing, social work, or related field; 2+ years of experience in community health or patient education; strong communication, cultural competency, and case management skills. Certifications in CPR or health education preferred. Flexible schedule, reliable transportation, and the ability to work with diverse families are required.
